STEP 1:   DOWNLOAD

One  link will take you to our Drop Box page.  The other link will take you to our Google Drive.  You can download from either place... the choice is yours.  You will open and then either download and print the form(s) or find the print icon and print the form(s) that you want.

STEP 2:  YES ANSWERS ON THE FORM

If you need to answer "yes" to a question on PAGE 1 then go to THAT SECTION on PAGE 2.  If there is a "YES" ANSWER ON PAGE 2 then you will need to TAKE ALL 3 PAGES to the doctor’s office for signature.  If you have seen the doctor within the last year, most of the time, the doctor will sign the form and you can pick it up the next day.  If there are only “no” answers then you will just need to sign the bottom of page one.

STEP 3:  PRINT IT, FILL OUT & BRING IT

Once printed, fill them out, and bring them with you to your first class session at Discover Diving.
